Why is vision health important?

A good eye exam can help you improve your eyesight and your health.

  • 50% of the U.S. population requires corrective lenses.

  • 90% of those who spend three hours or more per day working at a computer suffer from vision problems associated with eyestrain.

  • An eye exam can help provide early detection of major health issues, such as diabetes.

How does my plan work?

You will get the most from your vision benefits by visiting a UNUM doctor. UNUM's Network offers a wide choice of private practice optometrists, ophthalmologists and opticians. A UNUM provider can be located by visiting unum.visioncare.com or at Walmart, Sam's Club, JC Penneys, Sears Optical

If you visit an in-network provider for services and materials, all you have to do is identify yourself as a UNUM member to receive services. There are no claims forms to complete.

If you visit an out-of-network provider for services and materials, you'll be required to pay the full amount by that provider at that time. You can then submit a claim for reimbursement, which is a lesser benefit when compared to visiting a UNUM doctor.

Cost per Paycheck

Employee Only: $2.67/paycheck

Employee + Spouse: $5.33/paycheck

Employee + Child: $5.92/paycheck

Employee + Family: $9.27/paycheck


Employees may select this plan for themselves, spouse and children. (Children must be under the age of 26) This plan cannot be selected for spouse and/or children only. You, as an employee, must be enrolled in order to add spouse or children.

You can select this plan even if you do not select one of our medical plans.

Your insurance premiums are deducted through your bi-monthly paycheck each pay period. Make sure to have sufficient funds coming through your payroll account each pay period to cover the cost of your insurance selections.
